There are several ways to contact us:

If you want to make an appointment, click on the Make an Appointment button. This will put a message into our EMR (Electronic Medical Records), and we will contact you to make an appointment. You can use this whether you are a patient already, or not.
If you’re just wanting some information or to communicate with us, you can complete the “Contact Us” form below. This will go to our office’s email, and we will contact you, either by email or phone. Yes, you can use this form to ask for an appointment also.
If you are a current Pine Creek Family Medicine patient with a “Patient Portal” account, click on the button below. It is the most secure (HIPAA compliant) way to communicate with us.
You could send a Facebook message from our FB page, Pine Creek Family Medicine. But please be aware that is not a secure way to communicate.
